Integrative Center For Health ("we," "our," or "us") is a healthcare and wellness practice located at 315 Canyon Ave, Fort Collins, Colorado. We provide integrative, functional, and holistic health services under the direction of Nola MacDonald, CEO. By accessing our website (icforhealth.com) or using any of our services, you agree to be bound by these Terms & Conditions ("Terms").
2. Services
Integrative Center For Health provides a comprehensive range of wellness and aesthetics services including but not limited to:
Functional Wellness and Functional Medicine testing
ExoMind neurostimulation therapy
Emsella pelvic floor treatment
Emsculpt NEO muscle toning and body contouring
IV therapy (NAD IV, Saline Hydration, Immunity Boost, High-Dose Vitamin C, Glutathione, Myer's IV Cocktail)
Supplements and wellness programs
Other integrative health services as offered

3. Not an Emergency Service
Integrative Center For Health is NOT an emergency medical service. If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room immediately.
4. Appointment & Cancellation Policy
4.1 Scheduling: Appointments may be booked via our website at icforhealth.com/request-appointment or by calling (970) 292-5517.
4.2 Cancellations: We require at least 24 hours' advance notice to cancel or reschedule. Late cancellations may be subject to a fee as disclosed at the time of booking.
4.3 No-Shows: Clients who miss appointments without prior notice may be charged a no-show fee and required to prepay future appointments.
4.4 Office Hours: Monday through Thursday 10:00 AM to 6:00 PM. Appointments outside office hours are available by request.
5. Payment Terms
5.1 Fees are disclosed prior to services. By proceeding, you agree to pay applicable fees.
5.2 Payment Methods: We accept American Express, MasterCard, Visa, Discover, Google Pay, Apple Pay, Cash, and Check.
5.3 Financing: Financing options may be available through third-party providers. Financing terms are governed by the respective provider's agreement.
5.4 Refunds: Completed services are generally non-refundable. Prepaid packages are handled on a case-by-case basis.
6. SMS Messaging Program
Integrative Center For Health offers an SMS messaging program to opted-in clients. By opting in, you may receive:
Appointment reminders and confirmations
Wellness service follow-up notifications
Care coordination and service update messages
Promotional messages about new services, special offers, and wellness programs (marketing only with separate consent)
You can cancel the SMS service at any time. Text STOP to (970) 292-5517. After sending STOP, you will receive one confirmation message and then no further SMS messages. To rejoin, opt in again as you did originally.

Carriers are not liable for delayed or undelivered messages.
